Registration

If you want your child to receive services from us, you must first register your child.  Please fill out the form provided by your child’s school, or you can fill out a form online by using this Registration page.  You must fill out a separate form for each child.  Also, you must fill out a new form for each school year.

To sign up your child, find your child’s school in the lists below.  Then click the school name. 

Please note that following this link will take you away from the SC Smiles Dental web site and to a web site for Denticon by Planet DDS.  This is the website for the software that we use to manage all of our patient information.  It is a secure website and you can safely use it for patient information.

NOTE: When you have reached the Denticon registration page, please check the school name in the top right corner of the screen to make sure you are registering at the correct school. Only check the school name. The address listed is for SC Smiles Dental and is not the school address.
