If you want your child to receive services from us, you must first register your child. You must fill out a separate form for each child. Also, you must fill out a new form for each school year. There are several ways to register your child.
The easiest and fastest way is to send us an email at contact.scdsa@gmail.com. In the subject and body of the email, please state that you want to register a child for services. Include your contact information (your name, phone number and/or email). We will get in touch with you to get additional information and to complete the registration.
You can also get a registration form from your school or you can download the registration form and fill it out. You can then return the completed form to the school nurse or you can mail or fax it to us using the information on the form or on our Contact page. This process is likely to take longer than the method described in the previous paragraph.
